At Central Coast Direct Medical Care, we are proud to be led by Dr. Jardini, MD, and Reanna Clayton, FNP-C, who both have deep roots in the Central Coast community and are dedicated to redefining healthcare in our region. With over three decades of experience in family medicine, Dr. Jardini has built a successful practice by truly listening to the needs of his patients. Recognizing the demand for a more accessible and personalized approach to healthcare, he has partnered with Reanna Clayton, a nurse practitioner with a passion for community care.
Together, they have developed an innovative model of healthcare that prioritizes patient accessibility and individualized care. Our mission at CCDM Care is to ensure that patients can easily connect with their providers, eliminating long wait times for appointments and responses. We offer customized care plans tailored to fit your lifestyle, whether you prefer monthly visits, quarterly check-ins, or telehealth consultations. Timely communication is at the forefront of our practice, allowing us to swiftly address urgent care needs and ensure reliable follow-up for lab tests, home health services, physical therapy, and transportation.
Reanna, a Central Coast native, is committed to delivering exceptional care and fostering strong relationships within the community. With her diverse experience in emergency medicine and family practice, she shares Dr. Jardini's vision of providing a compassionate, patient-centered environment. Together, as founders of Central Coast Direct Medical Care, they pride themselves on offering an affordable and effective alternative to traditional clinic-based practices, ensuring that quality healthcare is accessible to everyone in our community.
Edward Jardini, MD
Dr. Jardini has been a dedicated physician to San Luis Obispo County patients for over 30 years. After graduation from the University of Soutern California School of Medicine, he pursued additional training earning a specialty in Family Medicine. A busy traditional office practice in Templeton, CA quickly followed.
Through the years, Dr. Jardini’s practice evolved to focus on the expanding needs of the Medicare-aged community. He attained board-eligibility status in Geriatric Medicine, and achieved certification in Hospice and Palliative Medicine.
Seeing an unmet need for bringing medical care directly to patients where they live, Dr. Jardini moved away from an office-based practice, and has aligned with Nurse Practitioner Reanna Clayton to establish Central Coast Direct Medical Care.

Born and raised on the beautiful Central Coast of California, Reanna Clayton has dedicated her career to providing exceptional healthcare to her community. She began her nursing journey in 2012 as an emergency department nurse, where she honed her skills in fast-paced, high-pressure environments and developed a deep understanding of patient care.
Driven by her commitment to advancing her expertise, Reanna pursued and obtained her Master’s degree in Nursing Administration and Family Nurse Practitioner. Over the past five years, she has transitioned into family medicine, where she passionately serves patients of all ages, focusing on holistic care and preventive health.
Reanna's dedication to her community shines through in her commitment to improving the health and well-being of those she serves, making her a trusted and compassionate healthcare provider.
Julie Pennington, Admin Lead
My name is Julie Pennington and I am proudly the administrative lead for Central Coast Direct Medical Care. I have had the privilege of working alongside Dr. Jardini for nearly two decades. Initially, I worked intermittently for the first twelve years before dedicating myself fully to managing his private practice for the past eight years. Throughout those early years, I explored various professional passions but ultimately returned to the medical field, drawn to the opportunity to collaborate with Dr. Jardini and Reanna Clayton.
Patient care remains my top priority, and I find immense fulfillment in assisting patients, actively listening and addressing their needs and always prioritizing compassion, patience, and understanding. I am excited to help build and grow essential services within our community along side Dr. Jardini and Reanna. It is an honor to be a part of the Central Coast Direct Medical Care team.
